Is your dog ready to join Planet K-9's Pack?

If you answer "NO" to any of the following questions, your dog may not be suitable to join Planet K-9's Pack:

  • If your dog is over 6 months of age, are they spayed or neutered?
  • Is your dog friendly when meeting new dogs?
  • Is your dog primarily indoors?
  • If your dog is over the age of 2, have they attended daycare before or visited a dog park?
  • Does your dog enjoy being around both big and small dogs?
  • Is your dog crate trained?
  • Is your dog ready for a day of fun?

We are very excited to have the opportunity to have your dog in our pack and commit to providing only the best care. Please provide some information about yourself and your beloved dog by completing the following 2 forms:

 
 

STEP 2: COLLECT YOUR VET RECORDS

Please ensure that your dog has had the following procedures within the past 3 years and that your vet records reflect:

• DHLPP (Titers accepted)

• Rabies

• Bordatella

• Negative Fecal

• Flea Preventative

• Spayed/Neutered (over 6 months old)

STEP 3: SCHEDULE YOUR EVALUATION

For the health and safety of all concerned, all dogs at Planet K-9 Daycare must pass a temperament test at time of evaluation to ensure that they are both dog and people friendly. 

Contact us to set up an evaluation for your dog to attend daycare. Bring your completed Registration Form and New Dog Profile, along with a copy of your dogs vet records, to your appointment. Also, a $32 nonrefundable fee is due at your evaluation.
